Required documents Bachelor's degree and Master's degree (long cycle studies)
Documents are submitted only by candidates qualified for admission. On the date of qualification, these candidates will be given the status of “qualified”, then the candidates qualified for admission to the first cycle studies (Bachelor’s degree) shall provide a complete set of required documents on the date indicated in the recruitment calendar. Please do not send the documents before the announcement of the qualification results.
The following documents should be submitted by accepted Candidates (after qualification results):
1) Personal questionnaire with an accepted photo – a printout, available for download as a PDF file on the candidate’s account at IRK in the “Enrollment applications” tab in the “Documents and further steps” section. (proof of identity must be presented for inspection when submitting documents in person). Before printing the personal questionnaire, check whether it contains errors and whether it contains a photo. The candidate must sign the printed questionnaire in the lower right corner in the space for legible signature.
2) Certified by SGGW: Copy (the original document must be presented) of secondary school leaving certificate – document entitling to undertake studies in the country of its issue, recognized in Poland by operation of law or under an international agreement, or by way of administrative decision issued by a Education Superintendent.
3) Administrative decision of Polish Educational Superintendent on the recognition of a secondary school leaving certificate obtained outside the Republic of Poland for a document confirming secondary education and the right to apply for admission to higher education (does not apply to certificates recognized by operation of law or under an international agreement).
4) Legalization or apostille of the secondary school leaving certificate obtained outside the Republic of Poland (does not apply to certificates recognized by operation of law or under an international agreement).
5) Certificate includeing information concerning the right to continue education at the next level in the country in which educational system a school operates, including the right to apply to higher education institution and the scope of rights appertaining. The aforementioned documents should be issued or certified by the school or educational institution which issued the certificate or they should be certified by educational authorities of the country which issued the certificate or in which system of education the school operates (does not apply to certificates recognized by operation of law or under an international agreement).
6) Copy of document confirming the knowledge of English (the original document must be presented).
7) Copy of valid passport (page with photo) or other document confirming the identity of the candidate (the original document must be presented).
8) Certificate from an occupational medicine doctor (applies to candidates qualified for admission to the fields of study for which this type of certificate is required). After admission, the accepted canditate is directed by the Faculty authorities for additional medical examinations to the occupational medicine doctor at the Polish medical facility, for this you have to print out „Skierowanie na badania” (referral for medical examination) which is available for download as a PDF file on the candidate’s account at IRK in the “Enrollment applications” tab in the “Documents and further steps” section. Accepted candidates are obligated to do this kind of medical examination as soon as possible after arrival to Poland and delivering a certificate to the relevant dean’s office.
9) 1 up-to-date photographs in accordance with requirements for the Polish ID (format 35 mm (width)/45 mm (height)) and the same digital photograph (resolution 500 x 625 pixels) attached to the application in IRK System
10) Declaration on health insurance during studying, declaration is available for download as a PDF file on the candidate’s account at IRK in the “Enrollment applications” tab in the “Documents and further steps” section.
11) Copy of certificate from consulate confirming Polish descent or Polish Charter, if the candidate holds one (the original document must be presented).
12) Translation into Polish of documents issued in languages other than Polish or English.
