Information for new students
SGGW Health Centre assists you with a wide variety of chronic health care problems including: asthma, hypertension, diabetes, high cholesterol and thyroid disorders. We encourage all students with chronic health issues to make an appointment with a SGGW Student Health Centre physician or nurse practitioner.
The SGGW Health Centre practices provide:
- appointments with doctor, practice nurse (GP and specialists)
- patient checks
- taking blood
- asthma checks
- general health checks
- blood pressure checks
- sexual health and contraception advice (Gynaecologist’s advice)
- screening for sexually transmitted infections
- breast examination advice
- travel advice and immunisation
- vaccinations
- stitch removal
- employment and insurance medicals
- ultrasound test
Patient Information
Have the following information available when you attend your appointment for the first time:
- details of your ongoing medical problems
- details of medical problems you have suffered in the past
- name of any medicines you are currently taking
- details of any allergies
- contact details of your registered or previous practice
Patient Responsibilities
Please advise us if you change your address, telephone number or medications.
Our practice teams do not accept any form of verbal or physical abuse.
Confidentiality
We collect information about you in order to provide the best level of care and treatment. We keep your information on file so that our doctors and nurses have the accurate and up-to-date information they need to help you. There are times when we have to pass on this information to hospitals, social services or the Health Authority. Please be assured that this is always done confidentially by removing non-essential identifying details. Everyone working for the NFZ has a legal duty to maintain the highest level of confidentiality about patient information.
Complaints and suggestions
In case of a complaint, please contact the Practice Manager on rejestracja@sggw.edu.pl or call our reception number – we will call you back.
We always strive to improve and enhance our services. Yet, if you have any suggestions, please call us
or use the suggestion box in our waiting room.
In less serious conditions, you can always seek healthcare at night or on public holidays where you will be given basic healthcare and assistance. These medical facilities are open from Monday to Friday from 6:00 p.m. to 8:00 a.m. the following day, and on Saturdays, Sundays and public holidays from 8:00 a.m. to 8:00 a.m. the next day.
